Effects of the replacement of hydrogenated oil by corn and grape seed oil on fasting blood sugar and lipid profile in type 2 diabetic patients

I.Introduction
Increase of blood sugar and lipid profile is the most important risk of cardiovascular disease in type 2 diabetes. Increase fasting Triglyceride and decrease blood HDL-cholestrol is the most prevalence disorder in these patients, also increase after meal Triglyceride level is common and a risk factor of cardiovascular damage in type 2 diabetic patients. Cardiovascular diseases is the major reason of death in about two third of diabetic patients. It has been estimated that diabetes prevalence that was 5.7 % in 2000 increase about 6.8 % at 2025 in Iran. Data from family survey shows that median of fat consume in Tehran is 29.8 % of total Energy, that is in the range of WHO recommendation whereas use of saturated fatty acids is very high, and the suitable way to decrease trance and saturated fatty acids intake is to replace them with mono unsaturated fatty acids (MUFA) and Poly Unsaturated fatty acids (PUFA). Because of the importance of lipid control in type 2 diabetes, this study is about the effect of replacement of hydrogenated oil by corn and grape seed oil on fasting blood sugar and lipid profile of type 2 diabetic patients.
II. Material and methods
Study was designed as a randomized cross-sectional clinical trial. 20 postmenopaused diabetic women (age <50 years) by HbA1c less than 10 % and didn't receive Insulin and without special diet were selected. Subjects randomly divided to two groups and clinical anthropometrical, diet, physical activity and biochemistry assessments has done. Amount and compound of fatty acids and purity percent in each oil were analyzed, in each group 10 subjects were allocated to replace hydrogenated oil by grape seed oil and corn oil respectively in the prime of study without any change in their diet, physical activity and receive any medicine. Hydrogenated oil consume usually in subjects at three weeks before the study, subjects intake replace oils in same amount of hydrogenated oil, by dietary assessment after 3 weeks washout then the groups were changed, biochemical assessment were done first and end of first stage, and once in the end of washout, 6 ml Blood sample after 12-14 hr fasting were collected from all subjects, fasting glucose, glucose Peroxidase, Triglyceride, Glotation Peroxidase, total Cholesterol and HDL-c were measured by enzymatic methods.
III. Statistical method
Food intake data and fat and oil table were analyzed by N3 software. Trans fatty acid introduced as saturated fatty acids. Data were analyzed by SPSS 15 software and quantitive variants in aspect of being normal were analyzed by Kolmogorov–Smirnov and use T-test for compare of quantitive variants between two diets, P<0.05 was introduced as significant. Because of two independent variants ( type and amount of oil ) on results of dependent variants assessed by repeated amount analyzing and note the amount of oil compare to helper variant and do Carrior Effect and Aeroid effect tests.
IV. Results
Anthropometric, physical activity, energy intake and dietary nutrient parameters didn't change significantly in different stages of study, Replacing of oils cause to increase intake of MUFA and PUFA significantly (p<0.01). Grape seed oil intake shows significant decrease on fasting blood suger (p<0.01) and triglyceride (P<0.01) compare to corn oil intake. But the level of after meal total Cholesterol, LDL-c, HDL-c and blood glucose do not show significant change compare to corn oil, because of good effect, intake of grape seed oil compare to corn oil on glucose and lipid profile, replacement of grape seed oil as a part of lipid intake in diet of type 2 diabetes patients is better than corn oil, but it require more studies.
